Below please find the link to the Reference form. You may complete the form on your computer and print the final version or you may print a blank copy and complete by hand.
Note: The format will not allow you to save a copy once you complete it on your computer, so be sure to print it once you have entered all the information.
If possible, please attach a photo to the reference form for recognition purposes.
If you prospective member lives in the Tampa area, you may send your reference form to Kathleen Lopez, 3324 W Sevilla Circle Tampa FL 33629.
If you prospective member does not live in Tampa, you should send it to her hometown alumnae reference chairman or directly to the collegiate chapter. Please email Kathleen for any of the collegiate chapter addresses.
